This commit adds `SubprocessReadableStream` interface that are used
for `ChildProcess.stdout` and `ChildProcess.stderr`. It's an extension
to a regular `ReadableStream` that provides convenience methods to collect
the output and parse it with `.bytes()`, `.arrayBuffer()`, `.text()` and
`.json()` helper methods (similarly to the ones available on `Response`).

This commits moves all `.d.ts` files from `ext/*` to `cli/tsc/dts`.
Due to TSC snapshot removal, `cargo publish` is now erroring out,
unable to find the declaration files. These files were moved to
"cli/tsc/dts", because it's much easier than keeping them in
extension directories, while still providing them compressed
or uncompressed depending on the build type.

When the body resource is unavailable when start reading it, the error
message is `Bad Resource ID` and that doesn't tell what's wrong very
well.
This PR changes that error message to `Cannot read body as underlying
resource unavailable`
This commit refactors how we access "core", "internals" and
"primordials" objects coming from `deno_core`, in our internal JavaScript code.
Instead of capturing them from "globalThis.__bootstrap" namespace, we
import them from recently added "ext:core/mod.js" file.
This commit renames "deno_core::InternalModuleLoader" to
"ExtModuleLoader" and changes the specifiers used by the
modules loaded from this loader to "ext:".
"internal:" scheme was really ambiguous and it's more characters than
"ext:", which should result in slightly smaller snapshot size.

This PR refactors all internal js files (except core) to be written as
ES modules.
`__bootstrap`has been mostly replaced with static imports in form in
`internal:[path to file from repo root]`.
To specify if files are ESM, an `esm` method has been added to
`Extension`, similar to the `js` method.
A new ModuleLoader called `InternalModuleLoader` has been added to
enable the loading of internal specifiers, which is used in all
situations except when a snapshot is only loaded, and not a new one is
created from it.

This commit adds a fast path to `Request` and `Response` that
make consuming request bodies much faster when using `Body#text`,
`Body#arrayBuffer`, and `Body#blob`, if the body is a FastStream.
Because the response bodies for `fetch` are FastStream, this speeds up
consuming `fetch` response bodies significantly.
