Fix wrong argument format in PyCodec_IncrementalEncoder() and

PyCodec_IncrementalDecoder().

Factor out common code from PyCodec_Encoder()/PyCodec_Decoder(),
PyCodec_IncrementalEncoder()/PyCodec_IncrementalDecoder() and
PyCodec_StreamReader()/PyCodec_StreamWriter().
